The European elderly care story has long been underpinned by one powerful fact: people are living longer, and the number of older adults is rising. That story remains true, but the market is changing in an important way. The easy narrative of “demographics equal opportunity” is giving way to a more demanding reality.
Today, growth is becoming more selective.
The view across Europe
Clariane’s FY2025 results show a major European operator recovering financial momentum, with €5.31bn of revenue, 4.5% organic growth and a renewed focus on cash generation and deleveraging. But just as striking as the recovery is where performance is coming from. The group pointed to balanced volume and price growth, notably in Germany, while its overall footprint still spans six countries and two major segments.
It can also be seen in real estate capital flows. Aedifica’s latest results underline that healthcare property capital remains active, with a €6.3bn portfolio, 618 properties, €293m of new investments in 2025 and an additional €276m pipeline. Yet even here, the emphasis is not indiscriminate expansion. It is disciplined allocation, portfolio rotation and strategic consolidation.
Carterwood research shows cost pressures in Great Britain

The same pattern appears in market data. In Great Britain, Carterwood’s trading and fee research shows strong fee growth but continuing cost pressure, with operators still balancing wage inflation, staffing pressure and occupancy performance. Planning activity has softened, and closures remain a real part of the market picture.
In other words, this is no longer simply a story about demand growth. It is a story about who can translate demand into sustainable returns.
That means asking harder questions. Where is supply genuinely constrained? Which assets are modern enough to command a premium? Which regions combine strong demographics with local affordability? How exposed is a business to labour inflation or reimbursement risk? Which markets are transparent enough to price risk properly, and which still reward deeper research?
