Spain is no longer an emerging story in elderly care. It is becoming the European growth market to watch

For years, conversations about elderly care real estate in Europe have tended to focus on the larger, more established markets. That is starting to change. Spain now looks like one of the most compelling places to watch.

Demographics, demand, and pricing

The ingredients are becoming hard to ignore. Spain’s population is ageing rapidly, with Carterwood’s latest research showing the number of people aged 65 and over rising from 10.2 million in 2025 to 14.3 million by 2039. Growth is even more pronounced in the older age groups that are most likely to need formal care.

At the same time, provision remains uneven. Coverage varies materially by region, and many southern and coastal areas still appear relatively underprovided. Even where development is coming forward, the pipeline looks selective rather than overwhelming. Carterwood’s analysis points to around 17,000 beds expected in 2026, but a much thinner picture after that.

That matters because the private market is already showing signs of strength. Carterwood’s first proprietary pricing dataset for Spain found average private monthly rates of €2,545 across its top-end sample, alongside average occupancy of 93.6%. Newer homes are commanding stronger pricing and are filling quickly, suggesting that well-designed product is meeting real demand rather than speculative demand.

Institutional capital has noticed. Threestones Capital announced in July 2025 that it had strengthened its presence in Spain with another nursing home acquisition. Aedifica, meanwhile, has continued to invest in Spanish healthcare real estate as part of a broader European strategy.

The bigger story is that Spain is still a market where information asymmetry is high. Compared with more mature markets, there is less transparent, standardised intelligence on private-pay pricing, local competitive dynamics and development feasibility. That creates risk, but it also creates opportunity.

For investors and operators who can read the market well, Spain offers something increasingly rare in European healthcare real estate: demographic momentum, fragmented supply, and demand for modern facilities to differentiate.

Spain is no longer just a market with potential. It is becoming a market where the evidence is starting to support conviction.
